Here's the part you won't like: rest is not healing you.
You know this story. Something starts hurting. You take two or three weeks off, the pain fades, you come back. A month or two goes fine, mileage climbs, you're already eyeing a race… and bang. Same spot. Same weekly mileage. Sometimes the same month as last year.
At this point most runners conclude "running isn't for me" or "I have bad knees." Wrong conclusion. The injury-rest-injury loop is not bad luck and not a verdict. It's a system with clear mechanics. And a system with clear mechanics can be broken.
Why the injury comes back: what's actually happening
Sports science has known two things about running injuries for a while.
First: the strongest predictor of your next injury is your previous injury. Not your weight, not your foot shape, not your shoes. The last injury. If "rest and return" actually fixed things, that statistic could not exist.
Second: load doesn't injure you. Load spikes do. Tim Gabbett called it the training-injury paradox in the British Journal of Sports Medicine (2016): high training loads by themselves don't break athletes; rapid changes in load break athletes, because tissues aren't ready for them. Muscles adapt in weeks. Tendons and bones take months. Your lungs are always ready before your ligaments are. That's the whole trap.
Now assemble the full loop:
You got injured. THEREFORE you rested, and during those weeks your tissues didn't "heal stronger," they detrained along with everything else. BUT your brain remembers the old you. THEREFORE within a couple of weeks of coming back you're at your old mileage, which for today's tendons is a massive spike. And the "out of nowhere" injury arrives exactly where you're weakest. Which is exactly where it hit last time.
Rest removed the pain. It never touched the cause.
Breaking the loop: 5 moves
- 1. Come back on a progression, not on feel. "Nothing hurts" does not mean "back to normal." After 3+ weeks off, restart around half your old weekly volume and add a little each week, with a lighter week every fourth. Boring? Extremely. The boring part is the part that works.
- 2. Add strength training. This one is not optional. A meta-analysis in the British Journal of Sports Medicine (Lauersen, 2014) found strength training cut overuse injuries almost in half. No stretching routine and no shoe technology comes anywhere near that effect. Two sessions a week, 20–30 minutes: squats, lunges, calf raises, glute bridges. That's the whole prescription.
- 3. Make easy days actually easy. A classic ingredient of the loop: you run few days, so every run becomes a proving ground. Your tissues never get a single recovery-level stimulus. I covered how to control intensity in Running by Heart Rate.
- 4. Check the foundation: sleep and food. Short sleep and chronic under-eating are the quiet accomplices in most "unexplained" injuries. Tissue rebuilds while you sleep. If you train on six hours of sleep and a permanent calorie deficit, your body repairs slower than you damage it. That math only ends one way.
- 5. Stop blaming your knees. Running itself doesn't wreck joints; recreational runners show less arthritis than sedentary people. I went through that data in Is Running Bad for Your Knees?. The knee that hurts is almost always the victim, not the culprit. The culprit sits upstream: the load spike, the weak muscles, the sleep schedule.

My own history with this
I went through my share of "out of nowhere" injuries back in professional sport. I remember the feeling well: fatigue that wouldn't lift, and anger at a body that "failed me again." The body never failed. The plan failed, because recovery in it was a footnote. A workout is just a stimulus; it breaks you down in the moment. You get stronger afterward, while you sleep, eat, rebuild. Runners who never accept that keep walking the same circle.
A comeback plan is the most delicate coaching work there is: volume progression, strength, honest easy days. It's exactly where solo attempts most often turn into the next lap of the loop.

Back to where we started: nobody exits the injury-rest-injury loop through rest. You exit through durability: slow volume growth, strength work twice a week, honest easy days, sleep.
Your body doesn't break from training. It breaks from training it wasn't prepared for.
